In 1967, the late Yoshihisa Miura, former president, opened the Miura Clinic.

1980: Established a geriatric hospital (20 years before the Nursing Care Insurance Act came into effect)

A system must be established that can respond appropriately to changes in drug prescriptions, etc., in the event of a drug supply shortage.

The patient must be fully informed that the medication administered may change depending on the drug supply situation and that any changes will be fully explained to the patient.

Regarding appointments

(1) When charging a special fee for appointments, it is necessary to ensure that a system is in place to enable each patient to receive medical treatment appropriately at the time of their appointment. If a patient is made to wait for more than a certain period of time (approximately 30 minutes) after their appointment time, it is not permitted to charge the appointment fee.

(2) Each medical department must ensure that at least 20% of the total outpatient consultation hours are set aside as time during which appointment fees are not charged. In securing such time slots, however, it is acceptable for each doctor or dentist in each medical department to have both appointment and non-appointment patients in the same consultation time slot.

(3) Patients without appointments will also be examined as appropriate, so as not to make them wait more than two hours.

(4) For patients with appointments, efforts will be made to ensure that appropriate consultation time (approximately 10 minutes or more) is available for charging a special fee for appointments, and the number of patients with appointments that each doctor or dentist can see in one day will be limited to approximately 40.

(5) With a view to providing appropriate information to patients regarding the above purpose, such information shall be posted within the hospital in a manner that is easy for patients to understand, and consideration shall be given to ensuring that such treatment is understood by both patients with and without appointments, using methods appropriate to each insurance medical institution, such as by dividing the reception desk of the insurance medical institution, providing explanations at the reception desk to patients without appointments, and distributing number tags to patients without appointments.

(6) The collection of reservation fees is permitted only for appointments made based on the patient's voluntary choice, and collection of such fees for the unilateral convenience of the hospital is not permitted.

(7) The amount of the reservation fee may be determined multiple times depending on the day of the week, time period, specialty, etc., but must be reasonable and appropriate from a social perspective.

(8) When determining or changing the content of special fees, etc., a report shall be made to the Director of the Regional Bureau of Health and Welfare (branch) each time using Attached Form 3.

(9) It is acceptable for there to be a physician or dentist who exclusively treats patients with appointments.

(10) Appointments may be made at night, on holidays, or late at night. However, in such cases, the overtime, holiday, and late night surcharges set out in the medical fee schedule or dental fee schedule cannot be calculated because the insured medical institution is prepared to treat the patient with the appointment.
